CCP Home Care
CCP Home Care Services
Our Community Care Program (CCP) is designed to help eligible older adults and individuals with disabilities receive the care and assistance they need while remaining safely and comfortably at home.
Through our collaboration with the Illinois Department on Aging, the Illinois Department of Human Services, and various Managed Care Organizations, we help connect qualifying individuals with important in-home support services. Depending on eligibility, these services may be available at little or no cost.
Who May Be Eligible?
Individuals interested in CCP services generally need to meet certain requirements, including:
- Being 60 years of age or older
- Living in the state of Illinois
- Being a U.S. citizen or qualified legal resident
- Meeting applicable financial and care-related eligibility guidelines
- Completing the Medicaid application process, when required
Eligibility and the level of care needed are evaluated by a local Care Coordination Unit (CCU). The assessment helps determine which services are appropriate and how many hours of care may be authorized.
Care Designed Around Your Needs
After approval, each client receives a care plan based on their individual needs and daily routine. Depending on the approved plan, support may include help with personal care, everyday activities, household responsibilities, and other essential tasks that promote independence and well-being.
In certain situations, clients may also have the opportunity to select a family member or someone they trust to provide their care. This option can help create a more familiar, comfortable, and personalized caregiving experience.

What Is Private Home Care?
Private home care is designed for individuals and families who prefer to arrange and pay for care independently rather than relying on government-funded programs. Depending on the situation, the cost of care may be paid directly by the individual or family, or may be covered in part by certain private insurance plans.
One of the main advantages of private home care is flexibility. Services can often be tailored to the individual's daily routine, personal preferences, and level of assistance needed. Eligibility is generally not based on age, income, or participation in a state-funded program.
Private Home Care May Include:
- Assistance with bathing, dressing, grooming, and other daily activities
- Meal planning and preparation
- Help with errands, shopping, and transportation
- Mobility and personal support
- Care schedules customized around individual needs
- The option to request consistent or preferred caregivers
- Flexible scheduling based on the family's lifestyle and care needs
Private home care is different from programs such as CCP and is generally not covered by CCP or Medicare for ongoing personal care, it may sometimes be used alongside other services. Depending on the individual's needs, families may also receive home health care or therapy services, such as physical, occupational, or speech therapy, through separate providers.
